Neck Lift in Ottawa

Neck Lift and Deep Neck Lift Planning

What is a neck lift? Neck lift surgery may address loose neck skin, muscle banding and changes in neck and jawline contour. In Ottawa, Dr. Sean C. Smith assesses skin, fat distribution, deeper support, chin projection and the relationship between the neck and lower face before recommending an individual plan.

Neck liposuction or neck lift? Neck liposuction removes selected localized fat. It does not correct substantial loose skin or every cause of fullness under the chin. A neck lift may address skin and underlying support, and deeper neck anatomy may affect the recommendation. The assessment determines which tissues contribute to the concern rather than treating every “double chin” in the same way.

Deep neck lift alone or facelift with neck lift? Neck concerns and jowls may need to be considered together. Dr. Smith discusses deep neck lift and extended deep plane facelift options according to the anatomy and goals. Ask what a neck-only plan would address, what it would leave unchanged and when combining face and neck surgery may be appropriate.

What does “mini neck lift” mean? This is not one standardized operation. Ask about the proposed tissues, extent of treatment, incision location, scars and limitations instead of comparing names alone. A smaller operation does not automatically provide the correction you want or eliminate recovery and risk.

How much does a neck lift cost in Ottawa? The published neck lift surgical fee estimate starts at CAD $15,000+, plus HST. The full quotation depends on the operation, anesthesia, facility care and any combined procedures. Confirm all included services, follow-up and additional costs after assessment; separate fee ranges should not be added to estimate a combined operation.

How should I plan recovery? Discuss swelling, dressings or drains, wound care, scar changes and individual restrictions for work, lifting and exercise. Review risks including bleeding, infection, altered sensation, nerve injury, scarring and possible revision. Obtain the clinic’s contact instructions before treatment.

Our surgical approach

Each lower face and neck rebalancing plan is developed as a surgical strategy, not a standardized package.

Careful assessment of skin quality, underlying tissue support, and facial proportions guides every recommendation. In some cases, limited intervention may be appropriate; in others, a more comprehensive approach is required. Additional procedures are recommended only when they meaningfully contribute to balance and longevity of results.

Restraint is central to this approach. The goal is refinement and harmony, not distortion.

What this procedure may address

Lower face and neck rebalancing may help address:

Loss of jawline definition

Soft tissue descent in the lower face

Changes in neck contour

Imbalance between facial regions

Every surgical plan is individualized and based on anatomy rather than age or trend.

Recovery After Surgery

When your procedure is completed, a bandage might be placed around your face to minimize swelling and bruising. Thin tubes may be present to drain any excess blood or fluid that may collect under the skin.

You will be given specific instructions that may include how to care for the surgical site and drains, medications to apply or take orally to aid healing and reduce the potential for infection, specific concerns to look for at the surgical site or in your general health, and when to follow up with Dr. Smith.

Be sure to ask Dr. Smith specific questions about what you can expect during your individual recovery period.

Where will I be taken after my surgery is complete?

What medication will I be given or prescribed after surgery?

Will I have dressings/bandages after surgery?

When will they be removed?

Are stitches removed? When?

When can I wear make-up?

When can I resume normal activity and exercise?

When can I see my stylist for hair color and cut?

Frequently Asked Questions (FAQ)

  • Understanding how the face and neck truly age

Facial aging is often described as a skin problem characterized by looseness, wrinkles, or sagging. In reality, aging is a structural process that occurs across multiple layers of the face and neck.

This understanding is foundational to how Dr. Smith approaches facial rejuvenation, and it is why he performs an extended deep plane facelift rather than skin-only tightening techniques.

    Aging happens in layers

The face and neck are composed of skin, fat pads, connective tissue, muscle, and underlying skeletal support. Over time, these layers do not simply loosen; they shift, descend, and change position.

    Common age-related changes include:

    • Descent of facial fat pads

    • Laxity of deeper support structures

    • Loss of definition along the jawline and neck

    • Changes in facial balance and contour

When aging is treated as a skin-only issue, correction is incomplete.

Why skin tightening alone falls short

Procedures that focus primarily on tightening skin may temporarily improve surface laxity, but they do not address the underlying cause of facial descent.

    Skin-only approaches often:

    • Create tension without restoring structure

    • Risk an over-tightened or unnatural appearance

    • Fail to produce durable, long-lasting results

    • Require greater tension on incisions and scars

    Natural-looking rejuvenation requires deeper correction.

The role of the deep plane

The deep plane of the face contains key support structures that determine facial shape, contour, and movement. By working within this layer, an extended deep-plane facelift allows the face and neck to be repositioned as a unit, rather than pulling the skin independently.

    This approach enables:

    • Restoration of facial fat pads to a more youthful position

    • Improved jawline and neck definition

    • Reduced tension on the skin and incisions

    • Results that move naturally with facial expression

Correction is achieved by repositioning, not stretching.

Why does this matter for the neck as well?

The neck is structurally connected to the lower face.

An extended deep-plane approach allows the face and neck to be addressed simultaneously, improving continuity between the jawline, chin, and neck. Treating the neck without addressing facial support often leads to incomplete or short-lived results.

Structural correction provides harmony rather than separation.

    Longevity through structure

Because the extended deep plane facelift restores deeper support, results tend to age more gracefully.

Rather than relying on surface tension, this technique respects how the face moves, settles, and changes over time. The goal is not to freeze the face, but to reset its foundation.

    Final perspective

Facial rejuvenation is most successful when it addresses the true mechanics of aging.

By treating aging as a structural process, not just a skin concern, Dr. Smith delivers results that look natural, feel balanced, and endure over time.

A private consultation provides the opportunity to discuss whether an extended deep-plane facelift or neck lift is appropriate and to plan rejuvenation with clarity, restraint, and long-term thinking.

  • Understanding when lifting restores more than volume

As the face ages, it is common to notice hollowness, softening of contours, and loss of definition. Because of this, many patients assume the solution is to add volume.

While injectables can play an important role, aging is not primarily a volume problem. It is a structural descent problem, and adding volume without addressing position can lead to imbalance over time.

This distinction is central to Dr. Smith’s extended deep plane facelift philosophy.

    What filler does well

Dermal fillers are effective when used selectively.

    They can:

    • Restore volume in specific areas

    • Soften transitions caused by mild volume loss

    • Support early signs of aging

    • Complement surgical correction when used conservatively

Used appropriately, filler can enhance balance without changing identity.

    Where filler falls short

As facial structures descend with age, adding volume alone does not restore position.

    Over-reliance on filler can:

    • Create heaviness in the midface

    • Blur natural contours

    • Distort facial proportions

    • Produce a swollen or overfilled appearance

This is often described as “looking filled, not lifted.”

Why lifting addresses the root cause

A facelift, particularly an extended deep-plane facelift, corrects aging by restoring facial structures to their natural positions.

    Rather than adding bulk, lifting:

    • Repositions descended fat pads

    • Restores jawline and neck definition

    • Reduces reliance on volume

    • Preserves natural facial movement

Correction comes from resetting the structure, not layering the product.

The danger of postponing lift with excessive filler

Filler is sometimes used to delay surgery beyond the point where it is most effective.

    Over time, this can:

    • Make surgical correction more complex

    • Mask true anatomy

    • Increase tissue heaviness

    • Lead to dissatisfaction despite repeated treatments

More filler is not a substitute for appropriate lifting.

How do we decide between lift and fill

The decision is guided by anatomy, not age or trends.

    During consultation, we assess:

    • Degree of structural descent

    • Skin quality and elasticity

    • Facial balance and proportions

    • History of injectable use

    • Desired outcome and longevity

In some cases, filler is appropriate. In others, lifting provides a cleaner, more natural correction. Often, restraint with filler following surgery yields the best long-term balance.

    A balanced philosophy

Dr. Smith does not view lift and fill as opposing treatments, but as tools with different roles.

The goal is not to do more, it is to do what restores harmony.

When structure is corrected properly, less volume is needed, the results look lighter, and faces age more naturally.

    Final perspective

Youthful faces are defined by position, not fullness.

By prioritizing structural correction through an extended deep plane approach, Dr. Smith helps patients avoid overfilling and achieve results that feel natural, balanced, and enduring.

A private consultation provides the opportunity to determine whether lifting, filling, or a thoughtful combination is most appropriate, without pressure and without excess.

  • Preserving identity while restoring structure

One of the most common concerns patients express when considering facial surgery is the fear of looking “lifted.” This fear is valid, and it reflects a misunderstanding of what thoughtful facial rejuvenation is meant to achieve.

Dr. Smith’s goal is not to make faces look like they've been operated on. It is to help them look rested, balanced, and recognizably themselves.

This philosophy guides his extended deep-plane facelift and neck-lift approach.

    What people mean by “lifted.”

When patients say they are afraid of looking lifted, they are often describing:

    • Over-tightened skin

    • Altered facial expression

    • A pulled or wind-swept appearance

    • Loss of softness or individuality

These outcomes are typically the result of skin-driven techniques or excessive tension, not of modern structural lifting.

What do we mean by natural

A natural result does not mean unchanged.

    It means:

    • Facial features remain familiar

    • Expression and movement are preserved

    • The jawline and neck appear refined, not forced

    • Changes are subtle and cohesive

People often notice that you look refreshed or well-rested, without identifying surgery as the reason.

Why structure matters more than tightness

Natural results depend on repositioning, not pulling.

An extended deep plane facelift allows deeper facial structures to be restored to a more youthful position. Because correction happens beneath the skin, surface tension is minimized.

This reduces the risk of distortion and allows the face to move naturally with expression.

    Avoiding the “overdone” look

The overdone appearance most patients fear is rarely caused by lifting itself.

It is more often the result of:

    • Over-tightening skin

    • Excessive volume replacement

    • Ignoring facial balance

    • Treating areas in isolation rather than as a whole

Restraint and anatomical respect are essential.

    How we define success

A successful result is not one that looks dramatic in the mirror.

    It is one that:

    • Integrates seamlessly with your natural features

    • Feels comfortable and expressive

    • Ages gracefully over time

    • Does not require explanation or concealment

    Subtlety is intentional.

    Final perspective

Facial rejuvenation should enhance identity, not replace it.

By prioritizing structure, movement, and restraint through an extended deep-plane approach, Dr. Smith helps patients look like themselves, just restored to a more balanced state.

A private consultation provides the opportunity to discuss goals openly and determine whether facial lifting is appropriate, without pressure and without exaggeration.

  • Choosing the right scope of correction

When considering facial rejuvenation, many patients ask whether they need a facelift, a neck lift, or both. These procedures are often discussed as interchangeable options, but they address different anatomical concerns.

Selecting the appropriate surgical scope is essential for achieving balanced, natural-looking results and avoiding incomplete correction.

    Face Lift

    What a facelift addresses

A facelift is designed to correct structural descent in the mid and lower face.

    It primarily improves:

    • Sagging of the cheeks and midface

    • Loss of jawline definition

    • Deepening folds around the mouth

    • Facial heaviness caused by descended fat pads

With an extended deep-plane approach, deeper facial structures are repositioned rather than relying solely on skin tightening.

When a facelift may be appropriate

A facelift may be recommended when:

    • Facial descent is the primary concern

    • The jawline has lost definition

    • The cheeks appear heavy or tired

    • Neck changes are mild or secondary

A facelift improves facial balance, but it may not fully correct neck laxity if structural neck changes are present.

    Neck Lift

    What a neck lift addresses

A neck lift focuses on the area below the jawline.

    It is designed to improve:

    • Loose or excess neck skin

    • Loss of neck definition

    • Visible muscle banding

    • Submental fullness

The neck is a complex structure, and surface tightening alone often produces limited or short-lived results.

When a neck lift may be appropriate

A neck lift may be recommended when:

    • Neck aging is the primary concern

    • Facial descent is minimal

    • Jawline structure remains strong

    • The face itself does not require repositioning

In carefully selected patients, a neck-focused approach can provide meaningful improvement.

    Combination Face and Neck Lift

Why the face and neck are often treated together

The face and neck age as a connected unit.

Structural descent in the face frequently contributes to neck changes, and treating one without the other can lead to imbalance or incomplete correction.

An extended deep-plane facelift allows coordinated rejuvenation of the face and neck, improving continuity along the jawline and neckline.

When a combination approach is recommended

A combined face and neck lift may be advised when:

    • Both facial descent and neck laxity are present

    • Jawline definition has been lost

    • Treating only one area would leave an imbalance

    • Long-term harmony is the goal

Combination treatment is not about doing more surgery; it is about addressing the full anatomical picture.

Why choosing the wrong scope causes problems

Under-treating aging by choosing too limited a procedure can result in:

    • Persistent laxity

    • Incomplete improvement

    • A mismatched face-to-neck transition

    • Desire for early revision

Conversely, over-treating areas that do not require correction introduces unnecessary risk without benefit.

    Appropriateness protects outcomes.

    How the decision is made

    During consultation, Dr. Smith evaluates:

    • Degree and location of structural descent

    • Skin quality and elasticity

    • Jawline and neck anatomy

    • Facial balance as a whole

    • Long-term aging patterns

Recommendations are made based on anatomy and balance, not age, trends, or procedure names.

    Final perspective

Face lift, neck lift, and combination approaches are tools, not tiers of severity.

The right choice is the one that restores proportion, preserves identity, and ages gracefully.

A private consultation provides the opportunity to determine the appropriate scope of rejuvenation with clarity, restraint, and long-term thinking.

  • Thoughtful planning, realistic expectations

Scars are an unavoidable part of facial surgery. Our philosophy is not to promise invisibility, but to plan incisions that heal quietly, respect facial anatomy, and preserve natural appearance over time.

Well-planned scars that heal properly result from technique, tension management, and patience, not marketing promises.

    How facial scars are different

Facial skin heals differently from skin elsewhere on the body. It is more visible, more expressive, and more closely tied to identity.

Because of this, scar planning for face and neck surgery requires:

    • Precise incision placement

    • Respect for natural creases and hairlines

    • Minimal tension on closure

    • Techniques that allow the face to move naturally

Scar quality is influenced as much by how surgery is performed as by where incisions are placed.

    Where incisions are typically placed

In an extended deep-plane facelift and neck lift, incisions are carefully positioned to follow the natural contours of the face and neck.

    These may include:

    • Along the hairline or within natural hair-bearing areas

    • Around the natural contours of the ear

    • Within existing creases, where possible

The goal is not to hide scars at all costs, but to place them where they can heal discreetly without distorting surrounding structures.

Why tension matters more than location

Scars are most affected by tension during the healing process.

Over-tightening skin to minimize scar length or force placement can:

    • Compromise the blood supply

    • Widen scars

    • Distort hairlines or ear position

    • Create unnatural pull on facial features

By correcting deeper structures through an extended deep plane approach, tension on the skin, and therefore on scars, is reduced.

    Understanding the healing timeline

    Facial scars evolve gradually.

Early scars may appear red, firm, or slightly raised before softening and fading over time. This process can take several months to a year.

Judging scar quality too early often leads to unnecessary concern.

    Our approach to scar care

Patients are guided through scar care with a conservative, evidence-based approach.

    This may include:

    • Protecting incisions

  • Resetting position, not stopping time

A face or neck lift does not stop aging. What it does is reset the starting point.

Understanding how results evolve over time is essential for setting realistic expectations and appreciating the value of thoughtful surgical planning. Dr. Smith’s extended deep plane approach is designed with long-term aging in mind, not just the immediate result.

How do faces age after surgery

After a lift, the face and neck continue to age, but from a more balanced, supported position.

Rather than fighting gravity with surface tension, structural repositioning allows tissues to settle and change naturally over time. This leads to aging that appears more gradual and harmonious.

Patients often look consistently refreshed, rather than dramatically altered.

Why does structure age better than tightness?

Results that rely heavily on skin tightening tend to reveal their age more quickly.

As skin relaxes over time, tension-based results can:

    • Lose definition unevenly

    • Emphasize scars

    • Create a pulled appearance

    • Require earlier revision

By contrast, an extended deep plane facelift restores deeper support. Because the skin is not bearing the primary load, results tend to remain softer and more stable over time.

    Longevity is influenced by planning

The durability of a lift is influenced by multiple factors, including:

    • Degree of correction required

    • Skin quality and elasticity

    • Surgical technique and tension management

    • Lifestyle factors such as sun exposure and smoking

    • Natural aging patterns are unique to each individual

Thoughtful planning during the initial surgery plays a significant role in how results evolve in the long term.

    Maintenance after a lift

Maintenance does not mean repeating surgery.

Many patients benefit from conservative, non-surgical care over time to support skin quality and overall facial health. When used thoughtfully, these measures complement surgical results rather than attempt to replace them.

Aggressive treatments or excessive volume replacement are rarely necessary, and often counterproductive.

    When revision may be considered

    Revision surgery is not inevitable.

However, over many years, some individuals may consider additional correction due to natural aging or life changes. When revision is considered, it is approached conservatively and strategically, building upon the existing foundation rather than starting over.

    A realistic perspective on aging

The goal of facial rejuvenation is not to look permanently young.

It is to look like yourself, just restored to a more balanced point in time.

Patients who understand this tend to feel more satisfied with their results and more at ease as they continue to age.

    Final perspective

A well-performed face or neck lift should age quietly.

By prioritizing structural correction through an extended deep-plane approach, Dr. Smith helps results remain natural, expressive, and proportionate over time.

A private consultation provides the opportunity to discuss long-term goals, aging expectations, and whether surgical rejuvenation aligns with your vision, without pressure and with clarity.

Step 1

Anesthesia

Medications are administered for your comfort during the surgical procedure. The choices include intravenous sedation and general anesthesia. Dr. Smith will recommend the best choice for you.

A variety of other procedures can further enhance the outcome of a facelift. They include:

Fat transfer

Soft tissue augmentation to recontour the facial structure

Wrinkle reduction by injection of fat

Step 2

The incision

Neck lifting is often combined with a facelift. The approach is individualized and discussed during consultation. When a facelift is combined with neck lifting, the following facial tissue adjustments may be part of the procedure. Fat may be sculpted or redistributed from the face, jowls and neck, and underlying tissue is repositioned, commonly the deeper layers of the face and the muscles are also lifted.

Skin is re-draped over the uplifted contours and excess skin is carefully trimmed away. An incision beneath the chin (submental incision) is usually used for neck lifting. The incision plan is tailored to each patient and discussed during consultation. Sutures will close the incisions.

Traditional Facelift:

A traditional facelift incision often begins in the hairline at the temples, continues around the ear and ends in the lower scalp.

Neck Lift:

Sagging jowls, loose neck skin and fat accumulation under the chin may be corrected with a neck lift. The neck lift incision often begins in front of the ear lobe and wraps around behind the ear ending in the lower scalp.

Step 3

Closing the incisions

Once healed, the incision lines from a facelift are well concealed within the hairline and in the natural contours of the face and ear.

Results

It may take a few months for swelling to fully dissipate and for incision lines to mature. Life-long sun protection and a healthy lifestyle will help extend the results of your rejuvenated, more youthful appearance.

As swelling and bruising subside, the visible improvements of a facelift appear. Your final result should not only provide a more youthful and rested appearance, but also help you feel more confident about yourself.

Although good results are expected from your procedure, there is no guarantee. In some situations, it may not be possible to achieve optimal results with a single surgical procedure and another surgery may be necessary.

Following your physician's instructions is essential to the success of your surgery. It is important that the surgical incisions are not subjected to excessive force, abrasion, or motion during the time of healing. Dr. Smith will give you specific instructions on how to care for yourself.
